Care & Support Team
365CPS Care Leaders offer care and support for cancer fighters who are going through the cancer journey. Our mission is to journey with every cancer fighter to provide relevant care and support in areas of psychosocial services so that they are connected to 365CPS as well as community resources to empower them to continue to be resilient in the fight against cancer. Our case management approach is briefly as follows,
Care and support
Home visitation
Telecare through phone calls, WhatsApp or zoom check-ins
Connect members to 365CPS psychosocial services or community resources
Financial assistance
Curate group sessions with members to foster peer support and knowledge building among like-minded members who are either fighting cancer or in remission

With the support of 365CPS, I have someone to walk alongside me in the hardest moments of this journey and share my deepest fears and pain.”
